EDI Enrollment Guide
Use the EDI Enrollment Guide to help you complete the EDI enrollment process. Answer the questions below to make sure you complete and submit the correct documentation in the correct order.
Step 1: Does your practice service one or more of the CGS Jurisdiction 15 (J15) states?
- Home Health & Hospice: CO, DE, IA, KS, MD, MO, MT, NE, ND, SD, PA, UT, VA, WV, WY & the District of Columbia
- Part A & Part B: KY & OH
Please visit MAC Websites, Secure Internet Portals, & Electronic Mailing Lists | CMS
to identify your Medicare Administrative Contractor (MAC).
Step 2: Did you receive your Welcome to Medicare letter from CGS Provider Enrollment or CMS?
Yes No
You must have an NPI and PTAN to start the EDI enrollment process.
Note: If you're the Authorized Official for your practice, please contact CGS Provider Enrollment for assistance.
Step 3: Do you have an EDI Enrollment Agreement on file? (required to submit electronic claims to CGS J15)
Yes No
Complete and submit the appropriate form:
- Home Health & Hospice: HHH EDI Enrollment Agreement Form & Instructions

- Part A KY: Kentucky EDI Enrollment Agreement Form & Instructions

- Part A OH: Ohio EDI Enrollment Agreement Form & Instructions

- Part B KY: Kentucky EDI Enrollment Agreement Form & Instructions

- Part B OH: Ohio EDI Enrollment Agreement Form & Instructions

Step 4: Do you want to enroll with electronic claims transactions?
Yes No
We encourage all providers to use the myCGS portal.
Please see the myCGS User Manual for details and step-by-step instructions on how to register (after the EDI Enrollment Agreement is approved and a claim is adjudicated in the Medicare system).
Step 5. Complete the EDI Application for electronic transactions:

Step 6: Do you need access to Direct Data Entry (DDE) or the Professional Provider Telecommunications Network (PPTN)?
Yes No
Step 7: Use the EDI Application Status Tool to check the status of your EDI application.

Step 7: Complete the DDE & PPTN Application/Reactivation form:
- Home Health & Hospice: DDE & PPTN Application/Reactivation
- Part A: DDE & PPTN Application/Reactivation
- Part B: DDE & PPTN Application/Reactivation
Once submitted, you'll receive 2 emails: confirmation upon receipt and a decision (approved or denied) once the application is processed.
Continue to step 8.
Step 8: Use the EDI Application Status Tool to check the status of your EDI application.
