Provider Enrollment Application Fee Amount
The Centers for Medicare & Medicaid Services (CMS) requires an application fee for institutional providers that:
- Initially enroll in Medicare, Medicaid or the Children's Health Insurance Program (CHIP);
- Revalidate their Medicare, Medicaid or CHIP enrollment; or
- Add a new Medicare practice location.
This fee amount applies to any of the above-mentioned Medicare enrollment applications submitted on or after January 1st through December 31st:

CMS defines an "institutional provider" as any provider or supplier that submits one of the following paper or associated Internet-based PECOS Medicare enrollment applications:
- CMS-855A
- CMS-855B (except physician and non-physician practitioner organizations)
- CMS-855S
