- Is it ever going to be set up to where you only have to get the security code once a day?

As of right now, CMS has the mandate set up that it is for each log in.

- We’ve received denials stating the cert period was not listed on the narrative. Why was this denied?

If the reviewer doesn’t have the date of the certification period on the narrative, then the reviewer is unable to tell which certification period the narrative is discussing.

- The face to face and narrative documents in the certification of terminal illness are one packet where the narrative is an addendum. Do the pages need to be numbered as one document?

Any time numerous pages make up one document and you should put page one of five, page two of five and so on, that will clarify that it is one document. In addition, if there is a multiple page document and it isn’t consecutively numbered and it signed signature, then each page would have to be signed. But if you always number the document in sequential order with the total number of pages on each page, then it is clear that it is just one document.

- We include the encounter note with our face to face but does the encounter note also need to be signed?

The encounter note does not have to be signed but it needs to show it was the person who performed the face to face. When you are submitting the encounter note that goes along with the face to face, I would strongly encourage you to put a title on it to say face to face. That isn’t considered altering the document. It’s putting the title on it.
