Opioid Treatment Programs (OTPs)

Section 2005 of the Substance Use-Disorder Prevention That Promotes Opioid Recovery and Treatment for Patients and Communities (SUPPORT) Act establishes a new Medicare Part B benefit for OUD treatment services furnished by OTPs on or after January 1, 2020.

Since CGS has been processing application for OTPs, we have seen a pattern of the most common reasons for delays in processing initial enrollment applications.  Below are the most common reasons.

  • Substance Abuse and Mental Health Services Administration (SAMSHA) certification
    • A copy of the certification received from SAMSHA is required.
    • SAMSHA website must show that the OTP is fully certified.  Provisionally certified OTPs are not eligible to enroll in Medicare.
  • Section 6 and Organizational chart
    • A copy of the OTPs organization chart is required.  It must include the name and title of key personnel of the OTP and the name of any central administration or larger organizational structure to which the program is responsible. 
    • The chart must also include all managing employees including the medical director and program sponsor.
      • All of these individuals must also be reported in Section 6 of the CMS-855B enrollment application.
  • Completed form CMS-1561 Provider Agreement
    • This form must be signed and dated by an authorized or delegated official of the OTP.
    • The signature must be handwritten or digitally signed.

In addition, please remember for electronic funds transfer (CMS-588) application, a voided check or bank letter confirming the account is required.
