Scalp Cooling for Treatment of Chemotherapy-Induced Alopecia
National Coverage Determination (NCD) 110.6 – Scalp Hypothermia During Chemotherapy to Prevent Hair Loss
establishes coverage for mechanical scalp cooling to reduce the risk of hair loss for patients who have alopecia as a common side effect to cytotoxic chemotherapy treatment regimens for breast, ovarian, fallopian tube, and peritoneal cancer.
This article provides instructions for physician offices and hospital outpatient departments to code these services.
CPT Codes
| Category III CPT Code | Long Descriptor | Billing Instructions |
|---|---|---|
| 0662T | Scalp cooling, mechanical; initial measurement and calibration of cap | Bill once per chemotherapy treatment period (i.e., If the course of chemotherapy treatment involves 6 or 18 sessions, report CPT code 0662T only once.). |
| 0663T | Scalp cooling, mechanical; placement of device, monitoring, and removal of device | Bill for each chemotherapy session throughout the chemotherapy treatment period in which the service is provided. Report separately in addition to the primary procedure code. |
- The cap is a supply that isn’t eligible for separate payment.
- The need to use scalp cooling for more than 8 sessions may be subject to medical review since most chemotherapeutic courses of treatment are 8 session or less.
ICD-10 Codes
For all sessions, use the most specific ICD-10 code to indicate active chemotherapy with an alopecia-inducing agent to treat breast, ovarian, fallopian tube, or peritoneal cancer.
Part A Hospital Outpatient Prospective Payment System (OPPS) Payment
| CPT Code | Status Indicator | Payment |
|---|---|---|
| 0662T | S | Separate APC payment |
| 0663T | N | Payment is packaged with other services. |
Part B Professional Services Payment
Category III CPT codes are a set of temporary codes that allow data collection for emerging technologies, services, procedures and service paradigms. CGS determines the payment rate for these services or procedures until Medicare establishes a national payment rate.
