Avoid CERT No Documentation Errors

CGS is seeing a rise in "no documentation received" errors for Comprehensive Error Rate Testing (CERT) reviews by the CERT review contractor (RC). The CERT RC  will send an Additional Documentation Request (ADR) to the provider or supplier that includes:

  • A documentation list of requested information for the service or procedure they're reviewing.
  • A barcode coversheet to submit on top of the requested documentation so it's easily identified and routed appropriately.

Responding to a request for medical records from any Medicare contractor is a CMS requirement as outlined in Social Security Act §1842(p)(4). The CERT RC is a separate contractor. CGS asks that you submit your documentation to them, so it doesn't delay your review. Failure to respond to your ADR will result in a claim denial and recoupment of the overpayment.

To avoid a CERT No Documentation error and recoupment of your claim payment:

  • Compile and photocopy all requested documentation for the service or procedure under review.
  • For lab or diagnostic tests, reach out to the referring provider. The billing provider is responsible for obtaining the medical documentation.
  • Place the barcoded cover letter on top of the documentation.
  • When faxing responses for multiple CERT ADR requests, send a separate fax transmission for each claim. Sending one fax for multiple claims can slow down the review process or cause a "no documentation received" error for a claim review.
