Important Reminder – List all HCPCS Codes on Prior Authorization Requests

If you submit Prior Authorization Requests, please list all the HCPCS codes you want reviewed on the Prior Authorization Request Coversheet. We can only review codes documented on the coversheet.

If you send requests by fax, email, or esMD, list every requested code on the Prior Authorization Request Coversheet to make sure they are reviewed.

If you submit orthotic codes through the myCGS® DME Web Portal, you can enter up to 3 codes per request. If you have additional orthotic codes to submit for Prior Authorization, submit a separate portal request to include the remaining codes.

Including all HCPCS codes on the coversheet helps keep the process consistent, accurate, and efficient across all CGS jurisdictions. It also helps prevent errors and unnecessary duplicate cases. Please check your internal processes to make sure every code needing prior authorization is listed on each coversheet you submit.
