Checklist for Home Office Cost Statement Submission

The following checklist is intended to help a provider ensure all required items are submitted with a Home Office Cost Statement (HOCS). Please ensure the following items are included with the HOCS submission:

  • Form CMS-287-22 must be filed for periods beginning on or after October 1, 2022. Form CMS-287-22 must be completed using software from an approved vendor and ECR and PI files must be submitted. This form is also e-signature capable and must be e-signed if submitting through MCReF.
  • A copy of the working trial balance to support expenses.
  • Documentation to support reclassification entries, adjustment entries and related party entries.
