Billing Instructions for Capecitabine Oral Anti-Cancer Drug
CGS has noticed that some suppliers are incorrectly billing Capecitabine using HCPCS code J8999.
Are you receiving ANSI CARC Code CO-16, ANSI Remark Code M119?
These claims do not have appeal rights. The claim is for an oral anticancer drug, but the National Drug Code (NDC) was missing, incomplete, or invalid. Claims for oral anticancer drugs must be submitted with a valid NDC.
How to Bill Capecitabine
For dates of service on or after January 1, 2025, follow the Oral Anticancer Drugs Local Coverage Determination (LCD L33826)
:
- Bill Capecitabine using the NDC instead of the HCPCS code.
- Make sure the NDC is effective on the date of service.
- Do not bill Capecitabine with a HCPCS J code.
What is NDC?
The NDC is a number which uniquely identifies a manufacturer's product in terms of the strength of each tablet, quantity of tablets in a package, and other packaging details. Suppliers must use the NDC that matches the product dispensed.
