Reopening Request Changes – Starting August 1

Starting August 1, CGS will no longer accept reopening requests by fax. This change will help us process your requests faster and more accurately.

New Reopening Form Required for Mailed Requests

If you send your reopening requests by mail, you must use the new Reopenings Form starting August 1. We will reject any requests that aren't on the new form.

Here's what to do:

Download the new Reopenings Form.
Review the new Reopenings Request Form Instructions
Type the information on the form – handwritten forms won't be accepted starting August 1.

We will accept both versions of the Reopening Form until August 1. Starting August 1, you must use the new form for all mailed requests.
