Amended Cost Report

A provider may file an amended cost report if they determine a correction needs to be made or additional information needs to be added to the filed cost report. However, an amended cost report must be filed before the review work has commenced for the applicable cost report. The following information must accompany an amended cost report:

  • Cover letter with a brief description and effect on reimbursement of each change made to the cost report. The cover letter should clearly identify the cost report as amended.
  • A new Electronic Cost Report (ECR) and Print Image (PI) file and a new signed and encrypted Worksheet S must be submitted.
  • Supporting documentation for the proposed changes.
