Example of ELGH Page 5

In this example, services provided to the beneficiary between January 1, 2012, and August 31, 2012, would need to be submitted to the MA plan. If you need to contact the MA plan (to update incorrect enrollment/termination dates or receive billing instructions), use the MA Claims Processing ContactsExternal Websitedirectory, which contains a listing of MA contracts and their corresponding plan type and plan ID.

Please be aware that MA plans differ from Medicare Secondary Payer (MSP) records in that a beneficiary's enrollment in an MA plan is an alternative to traditional Medicare benefits (Part A and Part B). Therefore, reimbursement for your home health services will either be from the MA plan (also known as Medicare Part C) or traditional Medicare. There are no "secondary" or "conditional" payments made by traditional Medicare contractors for an MA plan enrollment period as is the case for other types of insurance that may pay primary to Medicare Part A and B (auto/no fault, disability, liability, worker's compensation, working aged, etc.). MSP records (for insurers that may pay primary to Medicare) are displayed on ELGH page 4 and/or ELGA page(s) 14 and following.
