Top Claim Submission Errors for Home Health Providers: Error 11801
Reason for error: An invalid point of origin code (formerly known as source of admission code) was entered in the SRC field (FL 15) of the billing transaction. See the screenprint below that indicates where SRC field is found on FISS Page 01.
Most commonly for home health providers, this error occurs because a "B" or "C" point of origin code was entered for dates of service on or after July 1, 2010. A valid point of origin code is required on all billing transactions paid under the Home Health Prospective Payment System (HH PPS).

How to prevent/resolve:
- If the date of your home health billing transaction entered in the "STATEMENT DATES FROM" field (FL 6) is on or after July 1, 2010, the following point of origin codes are no longer valid:
- B
- C
- 7
