Need to Check the Status of Your Provider Enrollment Application?

Those who are listed on the Provider Enrollment application as the contact person, delegated or authorized official may call our customer service area to inquire on the status of (or any other information regarding) the application. If multiple people are privy to this information, please be sure they are identified on the application, as we cannot disclose information to anyone who is not.

The BEST way to obtain application status is by using the CGS Application Status Check tool. To use the tool, you must enter the reference number from the acknowledgement letter along with the 5-digit ZIP code of the contact address.
