During this webinar, providers will learn about the overarching elements of each of the following Medicare programs/services: Medicare Diabetes Prevention Program, Chronic Care Management, and Medicare Wellness Visits, and specific strategies for using them in Rural Health Care settings. The webinar will include videos and vignettes serving as case studies to provide learning opportunities for disease prevention, detection, and management.

Target Audience
This activity is intended for health care professionals engaged in the care of patients on Medicare in rural health clinics (RHC).
Educational Objectives
After completing this activity, the participant should be better able to:
- Explain the benefits of the Medicare Diabetes Prevention Program (MDPP) for patients who participate
- Demonstrate how to find a local MDPP by ZIP code
- Describe what Chronic Care Management (CCM) is and how it is helpful for patients and providers
- Explain the steps to start implementing CCM in practice
- RRecognize which billing code to use for each Medicare wellness visit
- Determine patient eligibility for a Medicare wellness visit
- Explain the clinical staff approach to implementing Medicare wellness visits and CCM
- Recognize which billing codes to use for CCM services and Medicare wellness visits conducted at an RHC
