Pre-Admission Screening
1. Was a Preadmission screening conducted by a licensed and/or certified IRF staff member within 48 hours prior to admission or, if greater than 48 hours, was an update made prior to admission?

1. Did the rehabilitation physician complete a complete assessment within the first 24 hours of admission (this time starts from the time the beneficiary arrives at the facility, not the time the admission orders are written) documenting the status of the beneficiary and any changes from the preadmission screening?

1. Was an IRF PAI completed and does it correspond with information in the beneficiary’s medical record?

1. Was the Overall Plan of Care completed within the first 4 days of admission and detail the beneficiary’s medical prognosis and anticipated interventions, including the intensity, frequency, and duration of interventions, as well as functional outcomes anticipated?

1. Does the Beneficiary require the active and ongoing therapeutic intervention of multiple therapy disciplines (MUST include either PT or OT)?
