Accreditation Denials – ANSI Code B7, Remark N211

Due to an error in the supplier accreditation files used by the DME MACs to process claims, certain claims submitted to the DME MACs may have denied in error with ANSI Code B7, Remark Code N211. The following messages will appear on your Remittance Advice for these claims:

B7: This provider was not certified/eligible to be paid for this procedure/service on this date of service. Note: Refer to the 835 Healthcare Policy Identification Segment (loop 2110 Service Payment Information REF), if present.

N211: Alert: You may not appeal this decision.

We are aware of these incorrect denials and are working with CMS and other contractors to correct the issue. Once the issue has been resolved, we will automatically adjust all claims that have denied in error.

Please note that if you provide DMEPOS items for which you are not accredited and have received B7 denials, then these denials may be correct. For additional information about accreditation, refer to MLN Matters Article MM7333External Website.
