You, your employees and agents are authorized to use CPT only as contained in the following authorized materials including but not limited to CGS fee schedules, general communications, Medicare Bulletin, and related materials internally within your organization within the United States for the sole use by yourself, employees, and agents. You shall not remove, alter, or obscure any ADA copyright notices or other proprietary rights notices included in the materials.

Forms Enhancement: The Ability to Save!

Based on your feedback, we have enhanced the CGS forms available on our website. You can still complete the forms electronically, then print and mail them to the appropriate department. Now, you can also save electronic copies of your completed forms. We encourage you to use this feature to save time and typing when submitting multiple forms. Please note: It is important to use the most current versions of CGS forms, which are always posted and available on our website. When we update our forms, we will notify you via our listserv.

It's very easy to save the form information- it's the same as saving any other file on your computer. Once you complete the information typed on the CGS form, simply click the "Save As" button (outlined in red) on the PDF file.
