NEW: Mailing Addresses Effective December 5, 2011
Effective December 5, 2011, CGS will be consolidating our mailroom locations to coincide with the final transition of our J15 segments. Please note the changes to our mailing post office boxes listed below. Also, please closely check that you are using the correct address for each inquiry type. When mail is sent to the wrong address for the inquiry type, it results in a delay in processing that inquiry/claim.
If you’re currently mailing to the “ Chattanooga, TN” addresses listed below, you will need to start mailing to the new address in Nashville, TN effective December 5, 2011. Until then, continue to use the current mailing address listed below.

Note: We are aware that some home health and hospice providers are still mailing information to the Cahaba addresses in Des Moines, Iowa. Those addresses were no longer valid as of June 13, 2011, for the “Inquiry Types” listed above. Providers should only be mailing information to the Des Moines address when directly instructed to do so by a CGS associate. Until December 5 th, the “Inquiry Types” listed above should be mailed to the Chattanooga, TN addresses noted under “Current Address Information” header. Please make sure that your staff is aware of the upcoming mailing address changes.
